A sports physical is more than just a regular check-up. It helps doctors find any health problems that could lead to injuries or affect how you play. These exams make sure you're safe and ready to join your team. You'll also get helpful tips on how to stay healthy and if any changes are needed in your training or activities.

What Is Required for a Sports Physical Exam?

To make sure your visit goes smoothly, please bring:

  • A copy of the IHSAA Sports Physical Form for us to review and sign.

  • Prescription glasses or contact lenses if your child wears them.
