Shelby County Public Library: Social Determinants of Health Library Section/Collection

$4,000 awarded

Library members at both the Shelbyville and Morristown branch now have access to works specifically referencing social determinants of health! These are also available digitally. MHP Providers will help select the items in these sections to better educate residents.

Shelby Senior Services: Hypertension Control Program for SSS Members

$3,800 awarded

Members at Shelby Senior Services can participate in a multi-week program that focuses on hypertension self-management. This is available at all four Horizon Centers in Shelby County!

This grant was a dual effort between the MHP Foundation and the Blue River Community Foundation.
