What You Need To Do

• Only complete one application type.
• Please fill out the application in its entirety. Incomplete applications will not be considered.

What To Expect From Us

• Please allow a two-week processing period. During this time, your application is reviewed and availability is determined.
• Each applicant will receive an email response within 3-4 weeks regarding their request. Applicants may receive an email requesting further information. Each applicant has 30 business days to respond or the application request is declined.
• Upon approval, each applicant will receive an email that contains instructions on how to make an appointment with MHP Education Services to complete the process.
